Chiari Malformation? Some core aspects of interest

Our first post in the new year recaps some bullet points on the Arnold-Chiari Syndrome Type I, which is often referred to as Chiari malformation.


The Arnold-Chiari Syndrome indicates the herniation of the lower part of the en-
cephalon and the cerebellum, the cerebellar tonsils, through the foramen magnum
towards the spinal canal.



- According to the WHO classification of diseases ICD 10, its name is Arnold-Chiari Syndrome.  

- Its most frequent form is the Arnold-Chiari Syndrome TYPE I, which is part of a new condition called Filum Disease.

- The Arnold-Chiari Syndrome Type I is far more frequent than one may think, more than 1 in 700.

- The Arnold-Chiari Syndrome Type I is not related to the action of the cerebrospinal fluid, but to the traction that the Filum Terminale ligament exerts on the whole central nervous system.

- The conventional treatment for the  Arnold-Chiari Syndrome Type I , the suboccipital craniectomy (decompression surgery) is contraindicated, as it causes more fatalities than the evolution of the disease itself.  
  
- Idiopathic Syringomyelia, which can be associated with Arnold-Chiari Syndrome Type I in the Filum Disease, is an injury of the spinal cord tissue due to lack of blood flow.

- The Arnold-Chiari Syndrome Type I can be associated to Idiopathic Syringomyelia, which in turn in 75% of cases is accompanied by Idiopathic Scoliosis. All 3 share the same cause together with other conditions, such as Basilar Invagination, Platybasia, Odontoid Retroflexion, Brainstem Kinking and others.

- The progression of the 3 conditions, (Arnold-Chiari Syndrome Type I , Idiopathic Syringomyelia, Idiopathic Scoliosis) and of the other clinical forms of Filum Disease, can be halted by applying the surgical sectioning of the Filum Terminale.
